Pimples hurt because the body is trying to get rid of the stuff that doesn't belong there. The redness, swelling, and inflammation cause the pain. The body knows that the dead skin, oil, and bacteria are supposed to be in the hair follicle (which is outside the skin).
Cystic acne is a type of inflammatory acne that causes painful, pus-filled pimples to form deep under the skin. Acne occurs when oil and dead skin cells clog skin pores. With cystic acne, bacteria also gets into the pores, causing swelling or inflammation. Cystic acne is the most severe type of acne.
Simply put, cystic acne causes pain due to the pimple's size, depth, and inflammation. Because they're so deep in the skin, they're closer to nerve endings, so high amounts of inflammation may be especially painful in certain areas.
Causes of Cystic Acne
Cystic acne occurs when bacteria, dead skin cells, and sebum (the substance that makes your face feel oily) get trapped beneath the skin's surface and become infected. This leads to a large, swollen cyst (bump) that can hurt just to touch.
Nodules are a type of hard pimple that can be large and painful. They form when an infected skin pore or follicle is located deep below the skin surface. Cysts are found deep below the skin when a pus-filled membrane forms around the infection. They are likely to scar.
Don't squeeze or pop.
Blind pimples are too far below the skin to pop. Trying will only irritate them further, and may cause infection or scarring. Instead of popping, try to bring the pimple to a head so it can exit the skin via other treatment methods.

Soak a clean washcloth in water that is hot, but not too hot to touch. Apply the warm compress. Hold the warm compress on the blind pimple for 10 to 15 minutes. Repeat the application three to four times a day until the blind pimple comes to a head and releases the pus.

Don't pop or squeeze pus-filled pimples
You can cause the bacteria to spread and the inflammation to worsen.
While ice can help reduce symptoms of an inflamed pimple, heat works well on noninflamed, blind pimples. A blind pimple is a type of closed comedo that develops in the deep layers of the skin.
"Pimples that really hurt are more than 'pimples'," says Dr. Ostad. "They are inflamed cysts — large pimples or boils. They hurt because they are more inflamed and the inflammation can be on top of or near a minor nerve ending which causes pain.
A pimple typically heals on its own in three to seven days. However, if you pop the pimple, it can become infected and take longer to heal. Topical acne treatments can shorten the amount of time it takes to heal.
Sanitize a needle or pin with rubbing alcohol. Gently prick only the very top of the whitehead with the tip of the needle. Do so on an angle that is parallel to the skin. Don't go so deep that you draw blood.
This means that by touching, prodding, poking, or otherwise irritating pimples, you run the risk of introducing new bacteria to the skin. This can cause the pimple to become even more red, inflamed, or infected. In other words, you'll still have the pimple, rendering any attempts useless.

Does putting toothpaste on a pimple make it go away? You may have heard this suggestion, but experts on acne say don't try it. Toothpaste could make that spot on your skin even more red, irritated, and noticeable.
Try steam or warm compress to speed up the healing process of acne. This also helps reducing pain and inflammation and relieving the pus. Soak cotton pad in hot water and press it against the pimple. Keep it that way for about 10 minutes and repeat a couple of times a day.
When to Pop, and When to Stop
If your pimple has a white or yellow head, it's prime for the popping. “At that point, it is OK to extract because the bump is very superficial to the surface of the skin,” says Lee.
Most blackheads are close enough to the skin's surface to attempt safe removal. If you've tried to remove a blackhead and the blockage won't come out, leave it alone for a day or two. In most cases, your skin will clear the blockage on its own if you give it time.
The technical term for an acne seed is a microcomedone. A microcomedone is a cluster of mostly dead skin cells that might be mixed with oil and comedogenic ingredients from pore-clogging products. It's called a micro-comedone because when it first forms, it is microscopic so it's invisible to the naked eye.
Milia are tiny, dome-shaped bumps on the skin that contain dead skin cells trapped in small pockets near the skin's surface. In some cases, milia are actually nicknamed “baby acne” or “Epstein pearls" due to their appearance.
"Sometimes when you remove contents from the skin and the skin is a little bit floppy, it can make noises," Lee explains in the episode.
If you're bleeding, she says to “gently blot the area with a clean tissue or cotton pad and clean the area with alcohol.” Once the blood has stopped, she advises applying a spot treatment containing benzoyl peroxide or salicylic acid as mentioned above.
