Why do dermal fillers cause swelling? Dermal Fillers commonly cause swelling and sometimes even bruising. The injection process disturbs the skin and soft tissue, causing minor trauma to the area, and swelling is a natural bodily response whilst the area heals.
After a few weeks you will lose some amount of volume due to the resolution of tissue swelling but daily fluctuations are inevitable based on how dehydrated you may be.

Yes Juvederm expands within the first few days. The hyaluronic acid is hygroscopic, meaning that it absorbs water from the surrounding tissue. That is why your lips can swell a bit if you have eaten a salty meal or are retaining water.
The hyaluronic acid in the filler binds with the skin's natural water supply, temporarily puffing up and swelling until the body reestablishes its natural balance. Again, the swelling is generally temporary and in most cases, lasts only a day or two.
If you don't receive routine maintenance your lip fillers will dissolve over time until it's completely metabolized in your body. Some people metabolize it very fast and will make the dermal filler dissolve fast. Others may metabolize the filler slower and may see longer lasting results due to that.

Keep in mind that swelling is a common side effect immediately following lip injections, so it is possible that what you're seeing is that swelling beginning to diminish.

The longevity of filler depends on many factors, like the type and amount, but, mostly, metabolic rate. Metabolism affects just how long filler lasts in each of our bodies, which is why your friend's lip filler might last longer than yours, or vice versa.
It might have to do with how your body metabolizes hyaluronic acid. There's no guarantee, but perhaps switching to a different brand of filler might help to get a longer lasting result. Speak to your injector/physician about this again.

Lip filler swelling stages in days
After the first day the bulk of any swelling should have subsided. By day 2 the initial swelling will have gone and you will start to see the full effects of the treatment. By this day the bulk of any pain should have gone, the primary swelling from the injections will have subsided.
As long as you gently place the cold compress on your lips and avoid applying excessive pressure (for example, don't try to hold the compress between your lips), it's perfectly safe to ice your lips. You can apply the cold compress for 5 –10 minutes at a time in the 24 to 48 hours after your injections.
Drink plenty of water and moisturise your lips to keep them as hydrated as possible. Avoid eating spicy foods that may irritate the delicate skin on the lips and keep touching to an absolute minimum while they heal.
Lip filler migration commonly occurs as a result of too much filler being injected into an area and “bleeding” elsewhere.
Over time, as facial movement increases in areas like the lips or cheeks, it causes the filler to break down and dissolve at a quicker rate. This makes the filler disappear faster in more active areas of the face.
