Topical steroids are grouped into classes based on their performance and concentration. 1 Class I steroids, such as clobetasol propionate, are the strongest and require a prescription; Class VII steroids, such as hydrocortisone, are the weakest and available over-the-counter (OTC).

Hydrocortisone is available both over-the-counter and by prescription, making it more accessible, while triamcinolone requires a prescription.
Dexamethsone is the most potent antiinflammatory CS and it has no sodium-retaining effects and a long duration of action. Prednisone and prednisolone are commonly used as anti-inflammatory agents, have mild sodium-retaining properties, and are equivalent in these actions.
One of the most common reasons for steroid creams not working is that not enough is applied. The package insert will usually say something like “apply sparingly” – but what does this mean? As a guide, the amount squeezed out from the tip of your finger to the first crease contains 0.5g of steroid cream.
This medicine is for use on the skin only. Do not get it in your eyes. Do not use it on skin areas that have cuts, scrapes, or burns. If it does get on these areas, rinse it off right away with water.
Using hydrocortisone for a long time without stopping can mean some of the medicine gets into your blood. If this happens, there's a very small chance it can cause serious side effects, such as adrenal gland problems, high blood sugar (hyperglycaemia), or problems with your eyesight.
For pimples & zits, hydrocortisone can start to work within 12 to 36 hours. For rashes, bites & eczema, it may take 2-4 days to see the effect of steroid preparations.
Combining these medications, especially over a prolonged period, may increase the risk of dehydration and hypokalemia, or low blood potassium. In severe cases, hypokalemia can lead to muscle weakness, paralysis, breathing and swallowing difficulties (due to muscle paralysis), and irregular heart rhythm.
Official answer. Topical clobetasol (Clobex, Impoyz, generics) is considered a very high potency topical corticosteroid (“steroid”) used on the skin, while triamcinolone (Triderm, generics) is a medium-to-low potency medicine.
The strength of the products ranges from 0.1% (1mg of hydrocortisone in each gram) to 2.5% (25mg of hydrocortisone in each gram). Pharmacies sell hydrocortisone skin treatments up to a maximum 1% strength. There is a stronger hydrocortisone cream called hydrocortisone butyrate (brand name Locoid).
How long does it take to wean off steroid cream? Talk with your doctor about how to stop your topical steroid use. After you stop their use, it can take anywhere from a couple of days to several months to see improvement in your skin. For a small number of people, this can take several years.
Do not apply hydrocortisone at the same time as other creams or ointments such as a moisturiser. Wait at least 10 minutes between using hydrocortisone and any other product. Try to use different skin products at different times of the day.
Steroid rosacea tends to be more likely to occur and more severe when strong steroids have been applied to facial skin. Conversely it is less likely to occur with mild steroids, particularly when applied infrequently.
How long will it take to work? Prednisone generally works very quickly — usually within one to four days — if the prescribed dose is adequate to reduce your particular level of inflammation. Some people notice the effects of prednisone hours after taking the first dose.

What happens if someone uses too much triamcinolone acetonide cream? If too much triamcinolone is used or a young child gets into the medicine, there are usually only minor symptoms such as stomach upset, nausea, vomiting, and diarrhea.
Official answer. You should not use topical corticosteroids like triamcinolone acetonide cream on your face, groin, genitals (private parts) or axillae (underarm, armpit) unless directed by your doctor.
