Retinoids such as Tretinoin, acids such as salicylic, and benzoyl peroxide are just a few of the products that cause purging. These products contain active ingredients that increase the skin cell turnover rate, therefore causing your skin to purge.
Many tretinoin users experience a “purge” during the first several weeks of treatment. During this period, acne — the very problem tretinoin is supposed to treat — often gets worse, resulting in everything from the occasional whitehead to severe breakouts.
The tretinoin purge means that the retinoic acid is working and that you are on the way to healthy, clear skin, possibly for the first time in years! If you're using tretinoin to treat the signs of aging, it's still possible to experience skin purging even if you don't normally suffer from acne or breakouts.
Some people will experience tretinoin purging when they first start using it. Tretinoin purging is when the skin gets worse before it gets better. Tretinoin speeds up the skin cell turnover process, causing some initial breakouts, drying, and peeling. These symptoms eventually go away and leave clearer skin underneath.
Skin purging happens when new ingredients, like retinol, promote increased cell turnover, which causes clogging and worsening breakouts. This is particularly the case as oil and debris that is trapped deeper underneath the skin comes to the surface.
Remember, that 'retinoid uglies' are likely to be temporary, and it will take time before you see the end result. You have three skin layers—the epidermis, dermis, and subcutaneous tissue. Your epidermis is your visible layer, and renews approximately every 28 days.
Skin purging typically looks like tiny red bumps on the skin that are painful to touch. They are often accompanied by whiteheads or blackheads. It can also cause your skin to become flaky. The flare ups caused by purging have a shorter lifespan than a breakout.
During the first 3 weeks you are using tretinoin, your skin may become irritated. Also, your acne may seem to get worse before it gets better. It may take longer than 12 weeks before you notice full improvement of your acne, even if you use the medicine every day.
Purging is slightly different, appearing on the skin mostly as blackheads or small skin-coloured bumps just under the surface of the skin. But it is also possible for purging to cause similar spots to a breakout, too.
In the first two weeks, your skin's oiliness will start to decrease. Before your first month of treatment ends, you may experience the “Accutane purge.” Skin purging looks like acne exacerbation.
How to treat skin purging. “If the skin barrier is compromised when you see purging then start ingredients which help with barrier repair, such as ceramides and hyaluronic acid in a non-comedogenic formulation. If you are using a treatment or product continue with a slower approach.”
Many have questioned whether skin purging is real. It may seem contradictory that continuing to use a product through breakouts and holding on through some serious bad skin days can result in your complexion eventually clearing. But purging is absolutely real—especially if you have acne-prone skin to begin with.
A sulfate-free cleanser, a skin calming moisturiser and a physical sunscreen during the day should treat the problem soon and take your skin back to its healthy and clear state. Also, during this time continue the product that's making your skin purge, don't worry it won't get worse.
Generally, it is fine to use tretinoin every night, but you may not want to because of the initial side effects, especially when you first start. You should only use tretinoin as prescribed to avoid significant skin irritation, redness, and other side effects.
While prescription-strength retinoids may have an effect in a matter of weeks, it can take up to 6 months for OTC retinols to produce the same results. You may notice a difference in conditions like acne after 12 weeks, but sun damage and signs of aging can take much, much longer to improve.
You might also get more breakouts once you start using retinoids. Keep calm and stick with it. “It's common to see acne get worse before it gets better, as the retinoids can cause a mass 'purge,'” says Robinson. Basically, as skin cell turnover increases, new clogs rise to the top.
Skin purging occurs because newly introduced skincare ingredients increase the rate at which your skin cells turnover, causing you to shed more dead skin cells than usual. This, in turn, pushes layers of dead skin off and also brings clogged pores to the surface, Chang says, resulting in more breakouts.
It may look like typical acne breakouts, but the conditions are different, with different durations, causes, and treatments. The good news is that purging isn't permanent and isn't there to stay for long. It's just the skin going through a cycle in response to certain ingredients applied to the skin.
Generally speaking, dermatologists say purging should be over within four to six weeks of starting a new skin care regimen.
Some people do experience worsening if they have long-standing acne. This is usually due to purging, in which the isotretinoin pushes out dead skin cells and debris. Because your skin can become red and dry, the medication sometimes makes acne look more inflamed and obvious.
Most patients don't see an appreciable difference until the third month of treatment. But first comes the “purge,” when all the old skin cells start rising—at an accelerated rate, with a potent retinoid—from the bottom layer of the epidermis up to the surface.
Accutane Side Effects:
Before your acne can get better, it will likely get worse at the beginning stages of taking Accutane. Called purging, the drug will push out all dead skin cells, oil, and debris from underneath the skin causing inflammation and more acne. Once this stage is complete, acne will begin to subside.
In order to avoid acne flares, many authors suggest to start oral isotretinoin treatment with a dose of no more than 0.5 mg/kg/ day for the first month, and then to increase the daily dosage to 1 mg/kg [7] .
If the acne is not improving then it may be that Isotretinoin is not being absorbed properly – so make sure you have a fatty meal with it. There are some people who do not metabolise Isotretinoin to the active drugs in the body very well and just need a higher dose.
