Pores are the tiny openings through which the oil, or sebum, produced by your body is transmitted to the surface of your skin. Your pores can become clogged for a number of reasons – excess oil resulting from hormonal changes, dry skin, a buildup of dead skin cells, or dirt and oil becoming stuck beneath the surface.
Due to an overgrowth of oil-producing cells, sebum can become trapped inside the gland, causing it to swell and form a bump under the skin. This is known as sebaceous hyperplasia. These bumps are harmless and often appear on the forehead and cheeks.
Though you may be tempted to squeeze or otherwise get rid of a sebaceous filament, it's best to leave them alone. Squeezing or picking at sebaceous filaments risks scarring and spreading any bacteria that may be in or around the pore to other parts of your face, causing a breakout.
You will notice that although they may look like little black dots from a distance, they are actually more of a white or yellow color. When you squeeze them, you'll notice a hard-ish clear substance (sebum) that resembles a thin 'filament'.
Clogged pores are caused by an excess of oil and dead cells mixing together in these spaces. The debris within a clogged pore can further develop into a blackhead (if it lies underneath the surface) or a whitehead (if it lies on the surface of the skin).
If you find your spots on the scalp are sebum plugs, they help to keep the hair shafts healthy, and thus the whole of your hair. They protect the hair and skin against bacteria, too, so it is important not to try to remove them, simply because they are there.
The lipid-laden cells in the sebaceous glands are wholly secreted (holocrine secretion) to form sebum. Triglycerides compose the majority of the lipid found in sebaceous gland cells. From the sebaceous glands, sebum drains into the hair follicle (see Fig. 2.11), from which it exits onto the surface of the skin.
High-dose red light photodynamic therapy (PDT) offers highly effective and durable treatment for acne by selectively destroying the sebaceous gland.

Always take caution as trying to extract sebaceous filaments can injure the skin and cause permanent scarring. It can also damage and stretch the pore, making it appear bigger.
Refined carbohydrates like sugar, refined flour, white bread, bakery products, desserts are quickly digested and absorbed into the bloodstream, resulting in a spike in insulin levels. High insulin levels increase the level of androgens, which stimulates excessive sebum production, oily skin and acne.
Vitamin D. Vitamin D is a fat-soluble vitamin that works alongside our sebaceous glands to regulate sebum production. Vitamin D helps our bodies protect against insulin resistance which can lead to oily skin as insulin stimulates the overproduction of sebum.
Vitamin B5
According to Dr. Dach, “Vitamin B5 works by reducing the oil production of the sebaceous glands.” Dach also recommends using L-Carnitine in conjunction with pantothenic acid, which can boost its effects.
