A tretinoin purge, also referred to as “tretinoin uglies” is a common skin reaction that occurs when a patient uses tretinoin (Retin-A or Tretin-X) for the first time.
For some users, acne will flare up within the first few weeks of using tretinoin before it starts to reduce acne. Tretinoin purge is also referred to as “acne flaring.” In addition to increased acne, a tretinoin purge can also include dryness, peeling or flaking skin.
The retinol uglies can last anywhere from 4–8 weeks. The exact length of a retinoid purge depends very much on your skin type and sensitivity. The amount and strength of your retinoid product can also play a part. Some people experience a purge flare-up for only a few days, and others don't get it at all.
Myers believes that “gentle exfoliation is your best friend” and can help to remove some of the dry, flaky skin that may be irritating you. Try to use a natural exfoliant such as a product that contains coffee, sugar, or oatmeal. Just don't scrub at your skin too hard!
The first 2-6 weeks of using Tretinoin generally involve your skin getting used to it. During this time, it's common to experience some skin irritation, purging, dryness and peeling. Although these are signs that the Tretinoin is starting to work not everyone gets them.
After 4-6 weeks, your skin will get used to Tretinoin and will begin to be noticeably smoother and more even. Depending on how often you use Tretinoin, you may begin to see full benefits by this point. A lot of the initial irritation will settle down and true “retinization” of your skin is in full force.
For those of us who do suffer from purging, it can look like: breaking out in pustules (pimples), the appearance of flaky, peeling skin, irritation and dry patches. Getting these symptoms can be demotivating, especially if you're already feeling unconfident about your skin.
Many tretinoin users experience a “purge” during the first several weeks of treatment. During this period, acne — the very problem tretinoin is supposed to treat — often gets worse, resulting in everything from the occasional whitehead to severe breakouts.

Tretinoin peeling is a side effect many people experience, especially in the first few weeks of using this medication. Tretinoin (Retin-A) can help reduce acne and brighten skin, but it also speeds up the skin's natural exfoliation process, resulting in dry and flaky skin.
Start small and increase slowly. When you first start using tretinoin, you may start with a lower percentage that you apply just two or three times a week. After two weeks, if your skin is adjusting well to the tretinoin, you can consider increasing use to every night as tolerated.
Initial Worsening: Some experience what looks like an initial worsening of their under eye wrinkles or dark circles. In fact, the skin isn't getting worse but the initial period of adjustment may include skin dryness, flaking or irritation makes it look rougher. This then makes the skin look more aged or tired.
Here's how to tell if your retinol has gone bad: Color Change: Fresh retinol products typically have a yellowish hue. If you notice that your retinol cream or serum has turned darker, especially brown, this is a sign that it has oxidized and is no longer effective.
Do not pick or squeeze the spots, as this can lead to inflammation or scarring. If this is Tretinoin purging or due to beta hydroxy acid (e.g. salicylic acid) or retinoid purging then you don't need to do anything else as these already treat acne breakouts.
Signs you should stop using tretinoin
It's normal to experience some dryness, peeling, and irritation—maybe even some purging—as you adjust to using tretinoin. But if the side effects become severe or don't go away, it might be time to hit pause and give your skin a breather.
Option one: The sandwich technique
After washing your face, apply a layer of moisturiser. Allow it to dry down for 5-10 minutes. Then, apply a pea-sized amount of tretinoin to your entire face. Allow this to dry down for 5-10 minutes.
Unless your doctor tells you otherwise, it is especially important to avoid using the following skin products on the same area as tretinoin: Any other topical acne product or skin product containing a peeling agent (eg, benzoyl peroxide, resorcinol, salicylic acid, or sulfur).
Tretinoin skin peeling is usually temporary and only lasts for a few months for most people, meaning patience is normally an effective approach. Use a moisturizer. It's totally okay to use a moisturizer with tretinoin. Apply the moisturizer first before tretinoin if you have skin dryness.
“During the skin purge, your skin speeds up the cell turnover faster than it's used to so the substances clogging your pores get pushed to the surface of your skin faster, so you may experience slight inflammation, contributing to additional breakouts,” says Dr. Fernandez.
Appearance. The blemishes caused by purging are usually smaller, come to a head quicker and heal faster. Breakouts, however, can vary widely in appearance, including blackheads, whiteheads, deeper cystic spots or hormonal acne along the jawline. They might heal slowly and can range in size and severity.
Using antioxidant serums can help. Vitamin c is the most popular serum but if you cannot tolerate it , you can use something with resveratrol and ferulic acid . Sunscreen is a must.
Can retinol damage your skin? If you use retinol, you'll sunburn more easily, which can damage your skin. Using high doses of retinol may also be associated with the development of skin cancer. Be sure to wear sunscreen and avoid direct sunlight (with clothing and hats) while you use retinol products.
If the skin purging stage is really troublesome, it can be helpful to decrease the frequency of your actives by alternating days of use. Stick with it - in time, things will likely improve. When the frequency of new spots starts to reduce, increase the frequency of your actives again.
