It is common to have more pain in the donor area than the recipient area after the operation. Some patients also report soreness in the donor area on the second healing week, this is completely normal as the tissue is recovering. Post-op hair transplant pain can be relieved with over the counter pain killers.
Pain in the donor area after hair transplant
It is relatively normal to feel some pain in the donor area after hair transplantation and follicles extraction, this feeling of pain and discomfort fades during the first few days after the operation.
After the bandage is removed, you should apply bacitracin or Neosporin ointment over the donor site (back of the head where the bandage covered) twice each day for the next 3-4 days. Do NOT apply an=bio=c ointment to anywhere in the front or crown where grafts were placed.

The skin graft donor area can be quite painful, as it is a very superficial wound. It is often more painful than the skin graft wound for the first week. This will get better gradually during the next couple of weeks. You will be given regular pain relief after the operation.
Itching is common during the healing phase. For the donor site ONLY, continue to apply Bacitracin or Polysporin twice per day on days two and three. Beginning on day four, apply Aquaphor ointment® two to three times per day to the donor site for up to two weeks until scabbing is no longer evident.
First Week: Swelling and redness typically subside within the first week. Patients are usually able to return to work and normal activities within a few days to a week. First Two Weeks: The scabs and crusts around the donor area typically start to fall off within the first two weeks.
Days 3- 6 following surgery:
Continue to apply a light layer of Vaseline or Aquaphor to the donor area daily. IMPORTANT: At 1 – 4 weeks post-op, you can expect that some transplanted hair will begin to shed. This is a normal process and should not be a concern as all the transplanted grafts will shed during this time.
Pain after hair transplant
Your pain may be worse on the evening of and the day after your surgery. The pain can be felt in your donor area but also where hairs have just been implanted. It is often described as a graze-like pain but shouldn't be severe. The hair transplant involves many small cuts to your head.
Hair from the pubic area is typically coarser and sometimes used for transplant purposes. Given its texture, it may be suitable for specific regions of the scalp where coarser hair is needed. However, it is not commonly used due to the sensitive nature of the donor area.
The average patient has ~ 6000 grafts available in his/her donor area for transplantation.
After we remove the bandage the next day, you can sleep in any position – over the back, sides or on the stomach. Additionally, you can lie on the donor areas too. However, for the first 10 days you should avoid letting the transplanted areas touch the pillow or anywhere for that matter.
In the initial days following the procedure, patients often report soreness, tenderness, and mild pain in the donor region. This discomfort generally starts to subside within the first week, with most individuals noticing significant relief by day 3-4 post-op.
Hair Transplant Procedure
After the anaesthetic wears off at 3-4 hours the scalp will feel a bit tight and sore but this soon eases and returns to normal in a few hours. Simple paracetamol or co-codamol will help. Minimal swelling is sometimes observed and is completely normal.
In particular, the "donor area" scar will begin to fade (become less visible) between 6 months and 2 years after the transplant - although it will probably never disappear completely. It is simply part of that hair restoration procedure's physical aftermath.
Itching occurs when damaged skin knits back together and is an entirely expected aspect of recovery following a hair transplant. Most clients experience itching at the donor site and around the transplant area as the microscopic incisions heal.
We ask that patients sleep on their back, with the head elevated on a few pillows. By raising your head during the night, the pillows decrease any swelling that can occur after the hair transplant. Patients are also advised to use the special pillow that they get at the clinic for the first 7-10 days.
Apply Neomycin ointment over the donor area for five days after the transplant procedure is complete to enable faster healing. The normal way of cleaning your hair, just like you are used to, must be resumed after no lesser than ten days from the day to the hair transplant procedure is complete.
Commonly recommended natural oils include coconut oil, almond oil, and olive oil. These oils have natural antioxidant, antibacterial, and anti-inflammatory properties, which help in reducing itching and enhancing recovery.
We advise that once the donor site and the skin grafted areas are fully healed, that you keep the area well moisturised and massage as often as you can. Suitable creams include E45, Vaseline Intensive Care, Aveeno, Nivea etc.
The donor area may have some hair transplant scabbing, swelling, and bruising in the days immediately post-surgery. Rubbing this or the recipient area could put you at risk of a hair transplant infection and set your recovery back.
Signs of infection: redness, warmth, unusual discharge or fever could indicate an infection that requires medical attention.
